How Much Protein Do You Need on Zepbound?

Zepbound (tirzepatide) delivers over 20% weight loss on average, but about a quarter of that can be lean mass. Here is exactly how much protein to eat on Zepbound, based on the latest clinical research.

Zepbound (tirzepatide) is the most effective weight loss medication ever approved in the United States. In the SURMOUNT-1 trial, adults taking the highest dose lost an average of 20.9% of their body weight over 72 weeks (Jastreboff et al., New England Journal of Medicine, 2022).

But not all of that weight is fat. A dedicated body composition substudy of SURMOUNT-1, published in 2025, used DXA scans to measure exactly what participants lost. The answer: roughly 25% of the weight lost on tirzepatide was lean mass, not fat. On average, participants lost 5.6 kg (about 12 lbs) of lean mass, a 10.9% reduction from baseline (Look et al., Diabetes, Obesity and Metabolism, 2025).

Lean mass includes your skeletal muscle, and losing it matters. Muscle drives your metabolism, protects you from injury, and is much harder to rebuild than fat is to lose. The two best-supported tools for protecting it are protein and resistance training.

How Much Protein to Eat on Zepbound

In 2025, four major US medical organizations, the American College of Lifestyle Medicine, the American Society for Nutrition, the Obesity Medicine Association, and The Obesity Society, published a joint Advisory on nutrition during GLP-1 therapy. Why Hitting That Target Is Hard on Zepbound

Tirzepatide is a dual GIP/GLP-1 receptor agonist. The SURMOUNT-1 substudy found the proportion of lean mass lost was similar across age and sex subgroups: nobody is automatically protected. Research consistently shows the combination of adequate protein plus 2–3 resistance training sessions per week is the most effective defense.
